v1.144.0 — A cancelled trip refunds the way each diver paid

Refund everyone on this trip now gives each diver's money back the way it came, in one press.

Card money back to the card — each card payment is refunded at the bank, one after another, with the Belize dollar amount shown beside it.
Account money back on the account — what a diver paid from their account goes back onto it, even with Credit switched off.
One choice for the rest — cash and other payments go back as one method you pick for the trip, starting on how most of it was paid.
Nothing held up by one card — if the bank doesn't confirm a card, the others still go through, and that diver's row keeps a Try the card again.
One receipt per diver — each diver is emailed once their money has all gone back. Other options… keeps the old one-method refund.
