The medical questionnaire is answered the night before
The guest answers the ten questions on their own phone. A yes asks for a physician's approval before the seat is secured, and the answers come back as the filled forms.
Without it
The Diver Medical is a clipboard the guest fills in five minutes before the boat leaves. A yes is discovered at the dock, where there is no physician and no time, and the shop is choosing between a refund and a risk.
What Seascape does
The questionnaire goes out with the booking and is answered on the guest's phone: ten questions, with follow-up boxes A to G where one is needed. All no, and the guest signs and is finished. A yes online stops the seat being secured and says a physician has to approve them; at the desk, staff record the approval they have been shown and carry on. The answers come back as the filled Diver Medical form 10346, and a student's record as PADI product 10056 or an SDI student folder.

Waivers and check-in
The waiver goes out with the confirmation, is signed on the guest's own phone, and is on their record before the tanks are loaded. The version of the text they agreed to is stored with the signature.
