Every booking shows how it was made

Reservations now say how each booking came in and who made it, so you can see at a glance whether a customer booked online, used one of your team's sign-up links, or was entered at the office.

On the Reservations list — each booking reads Online, Online · Ilya's link, or Office · Ana.
On the booking — one line under the Booking ID says who made it and when, and for a sign-up link which trip it was for and whether it was the QR code or the link.
Sign-up links and QR codes — they look and work exactly as before, and the customer gets the same emails as any online booking. Links sent out before today still work and show as Online.
